Life insurance company Aviva ¿ivotní pojiš¿ovna is to send out “Confirmation of Premium Paid for Private Life Insurance” (hereinafter referred to as "Confirmation") for all policies complying with the requirements set under Act No 586/1992 Coll, on Income Tax, as amended.
Confirmation will be sent both to policy-holders and to any employees who pay contributions to the policies. Confirmations will be sent out starting from Monday 25 January 2010 with all Confirmations to have been sent out by the end of January 2010.
The Confirmation is based on the recommended model issued by the Czech Ministry of Finance and ¿AP, the Czech Association of Insurers, and contains all the details required for claiming tax benefits (together with the insurance policy or certificate as evidence of the insurance policy).
Employers are not required to provide confirmation under the terms of the act and the confirmation merely offers employers a better overview for contributions made in the year in question rather than formal evidence.
The amounts specified in the confirmation correspond to the contribution made by the policy-holder/employer to the private life insurance policy, which is different in many cases from the total premium paid. It is impossible to use premium paid, for supplementary insurance in particular, or other insurance components, to obtain tax benefits and therefore, for the purposes of the Confirmation, it is deducted from the total premium paid.
If you have any queries or doubts relating to the Confirmation or the conditions for application of the tax benefit, please contact the Customer Services Department at Aviva ¿ivotní pojiš¿ovna (800 128 482).
